This skull is scientifically known as Stegodon trigonocephalus, which belonged to a juvinile Stegodon, at age around 12-14 years old at the time of death.
It has restorations at the tusks (they typically fall out at time of death) and at both zygomatic bones.
This piece is truly a unique museum quality !!! Add to this that skulls of juvenile Stegodonts are much rarer than those of adult animals due to their fragility.
